Main duties of the jobThe following are the core responsibilities of the Clinical Pharmacist delivering health services.
- Work as part of a multi-disciplinary team in a patient‑facing role to clinically assess and treat patients using your expert knowledge of medicines for specific disease areas.
- Care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co‑morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism (through STOMP Stop Over Medication Programme).
- Provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the organisation and to help in tackling inequalities.
- Through SMR's, support patients to take their medications to get the best from them, reduce waste and promote self‑care.
- Consult patients within defined levels of competence and independently prescribe acute and repeat medication.
- Receive and resolve medicines queries from patients and other staff.
- Provide medication review services for patients in the practice and during domiciliary visits to the local nursing home.
- Manage a therapeutic drug monitoring system and the recall of patients taking high risk drugs, i.e., anticoagulants, ect.

The Wilson Practice is a friendly, mutually‑supportive, happy two Partner and thirteen Associate GP Partnership covering 17,250 patients in a beautiful part of Hampshire. We have a strong Multi‑disciplinary team with ANPs, Triage nurses, Treatment room team as well as additional PCN role support for the daily service we provide.

The team is evolving and broadening to produce a sustainable, effective service for our patients, and a good working environment for our staff.
We run a total triage system using Accurx with 15 minute telephone and 15 minute face‑to‑face appointments. Our fantastic triage team run daily minor illness/triage clinics, and lead the Minor Injuries Service in our well‑equipped Treatment Room. Our Nursing Home ward rounds are completed by a dedicated ACP and GP partners. We have a full team of Admin staff including GPAs via our PCN.
We are fully engaged with our functioning Primary Care Network, which provides, remote GP for medication reviews, Physiotherapists, Social Prescribers, child and young person and adult mental health workers amongst others.
