Medical Assistant

Job Summary

The Medical Assistant (MA) - CCM plays a critical role in supporting our client's Chronic Care Management program by conducting structured monthly outreach calls and text message communications to enrolled patients. This role focuses on patient engagement, care coordination support, documentation, and escalation of clinical concerns to licensed providers. The MA serves as a consistent point of contact for patients, helping improve adherence, outcomes, and overall quality of care for individuals with chronic conditions.

Key Responsibilities

• Conduct scheduled monthly CCM phone calls to enrolled patients (75% of Time)
• Send and respond to patient text messages for follow-up, reminders, and engagement
• Build rapport and maintain consistent communication with assigned patient panels
• Educate patients on chronic conditions, care plans, and preventative health goals
• Reinforce medication adherence and lifestyle recommendations
• Collect and document patient-reported updates including symptoms, medication, compliance, and recent hospitalizations or ER visits
• Identify gaps in care and escalate concerns to licensed providers
• Assist with appointment scheduling, referrals, and follow-up coordination

Schedule/Shift

• M-F 9 am - 5 pm

Pay: $18-22.00/hr Depending on Experience

Required Qualifications:

• Certified Medical Assistant (CMA or RMA) preferred

• High school diploma or equivalent required

• Minimum 1 year of clinical or patient-facing healthcare experience

• Strong phone and text-based communication skills

• Basic understanding of chronic disease management

• Proficiency with electronic health records

Preferred Qualifications:

• Experience working with Medicare patients

• Prior experience in CCM, RPM, or care coordination

• Knowledge of CMS CCM documentation requirements

• Bilingual (English/Spanish) a plus
