We are seeking a Customer Success Manager (CSM) to lead and grow our Care Partners (CP) customer base. CPs are specialized partners who deliver care management services on behalf of providers and represent a strategically significant customer base for Chronic
CareIQ. This role is critically important: you will be responsible for partner success, retention, and measurable revenue growth. The ideal candidate is a hybrid of customer success manager and revenue generator, skilled in building trusted relationships while consistently identifying and capturing new commercial opportunities.
- Partner Relationship & Success
Own the Care Partners portfolio, serving as their trusted advisor and primary point of contact. - Lead QBRs, executive check-ins, and in-person meetings to strengthen relationships and uncover growth opportunities.
- Develop and execute tailored business plans with each partner, aligned to revenue expansion and program adoption.
- Build strong executive relationships to unlock strategic and commercial opportunities.
- Revenue Growth & Expansion
Drive upsell and cross-sell of CCIQ solutions across the full spectrum of care management programs (e.g., CCM, RPM, PCM, RTM, BHI, TCM, and future offerings). - Own revenue growth targets within the CP segment, working closely with Sales on pipeline strategy and deal closure.
- Identify whitespace in partner networks and translate it into measurable revenue.
- Equip CPs with sales enablement tools, training, and pro-forma ROI models to accelerate their own sales success.
- Lead workshops and joint sales activities, ensuring CPs are confident selling CCIQ to new providers and sub-accounts.
- Enablement & Marketing Support
Deliver sales workshops, webinars, and best-practice sessions to help CPs effectively sell CCIQ's value proposition. - Partner with Marketing on co-branded initiatives (spotlights, case studies, events) that generate demand and closeable opportunities.
- Support partner-led pipeline development, ensuring CCIQ provides clear ROI and closing support.
- Recognition & Retention
Launch and manage the Premier Care Partners Program, with benefits including advisory council seats, beta access, awards, and recognition. - Track and improve partner satisfaction (NPS) and engagement while mitigating churn through proactive action.
- Position CCIQ as a long-term strategic ally by making the partnership too valuable to replace.
- Revenue growth and upsell from Care Partners (primary KPI).
- Expansion of programs across the partner base.
- Sub-account and patient volume growth.
- Partner churn maintained below target threshold.
- Partner satisfaction (NPS above target score).
- Number of Premier CPs established by year-end.
- Engagement in events, workshops, and enablement programs.
- 57 years in Customer Success, Partner Management, or Strategic Account Management, ideally in healthcare SaaS or EHR environments.
- Proven track record of driving revenue growth, upsell, and expansion in a partner or channel model.
- Experience balancing commercial accountability with relationship management.
- Strong communication and executive presence with the ability to influence senior stakeholders.
- Skilled at sales enablement, workshops, and co-selling with partners.
- Analytical and data-driven; comfortable owning a revenue number and KPIs.
