Een zorgorganisatie in Almere is op zoek naar een enthousiaste casemanager dementie. In deze rol bied je educatie en advies aan cliënten met dementie en hun naasten. Je werkt zowel zelfstandig als in teamverband en hebt de verantwoordelijkheid om de kwaliteit van leven van cliënten te waarborgen. Een afgeronde HBO-V opleiding en BIG registratie zijn vereisten.
