Overview
The Cardiopulmonary Rehab Therapist helps to maintain the operations of a comprehensive cardiopulmonary rehabilitation program, to provide high quality patient care in keeping with the standards and strategic objectives as put forth by My Michigan Health, Rehabilitation Services; under the direction of the CR/PR medical director(s) and/or the CR/PR health system manager.
The Cardiopulmonary Rehab Therapist is responsible for participation and decisions in daily operations within the cardiopulmonary rehabilitation environment. This includes the development, implementation and regulatory compliance associated with patient care and the entire range of services provided in CR/PR such as: patient evaluation/assessment, patient engagement, patient interventions as indicated, patient and community education, exercise training and treatment, psychosocial support and continued reevaluation and follow up in conjunction with the CR/PR program's medical director.
Responsibilities- (55%) Daily operations and direct patient care consisting of supervised monitored sessions for the cardiac and pulmonary rehabilitation patient, under the supervision of the CR/PR medical director(s). Direct session and patient oversight for all patients utilizing respiratory service codes. Duties may include patient assessments, monitoring patients for physiological changes during session, ECG interpretation, and emergency response to untoward events. The Exercise Physiologist will also be responsible for the development, assessment/re-assessments, modification, and submission of the patient's Individualized Treatment Plan (ITP) to the Medical Director.
In conjunction with the ITP the exercise physiologist will assist the patient in maintaining the treatment plan by providing patients with assistance in exercise training, education on health improvement and behavior modification, provide patient with feedback and program outcomes, and communicating any deviations from the plan to the patient's provider. - (15%) Indirect patient care such as scheduling, insurance verification, billing, EMR work queues, and physician communication.
- (15%) In coordination with the education department, Medical Director, and Cardiopulmonary Rehab Manager reviews, maintains, updates Emergency Policies and coordinates training and mock codes for compliance. Will work with Cardiovascular Services and/or Respiratory Care teams to educate staff and perform quality improvements measures in patient education and referral process, program certification and national program registry. Leads and/or participates in Health System, Rehabilitation Services, and CR/PR program performance improvement initiatives to maintain the highest level of program quality and patient safety.
Actively participates in the Rehabilitation Shared Governance model and the CR/PR Shared Governance Work Groups. - (15%) Maintains patient's chart and performs chart auditing. Performs patient outcome assessments pre/post treatment and as indicated. Performs outcome data collection and interpretation for national program certification and national program registry. Leads and/or participates in Health System, Rehabilitation Services, and CR/PR program performance improvement initiatives to maintain the highest level of program quality and patient safety. Actively participates in the Rehabilitation Shared Governance model and the CR/PR Shared Governance Work Groups.

DUTIES AND RESPONSIBILITIES:
Responsible for obtaining: patient history, physical assessment measurements during exercise treatment, such as exertion, dyspnea, pain, blood pressure, heart rate, oximetry reading, heart sounds, blood glucose levels and interpretation of ECG rhythms. - Performs patient monitoring for inappropriate responses to exercise, discontinuation of treatment and appropriate reporting, as indicated to the CR/PR supervising physician, medical director and/or the patient''s referring physician.
