Pay Range
Base pay $32.75/hr - $35.00/hr
Job DescriptionThe Wellness Nurse supports resident health, collaborates with community nurses, caregivers, and medication staff to ensure compliance and clinical excellence.
ScheduleFull Time / Part Time, Days and Hours vary, every other weekend commitment.
Responsibilities- Coordinate resident admission and care planning.
- Deliver care per resident care plans.
- Administer medication per physician orders.
- Coordinate services with out‑of‑hospital providers.
- Document care and observe resident changes.
- Maintain knowledge of state regulations and policies.
- State regulations knowledge for compliance.
- Graduate from accredited practical nursing program.
- Current LPN or RN license and state registration.
- Minimum three years assessment and geriatric nursing experience (preferred).
- Compassion and eagerness to learn.
