Location:

Salary Range: $65,000 – $80,000 + Benefits

Schedule:

Monday – Friday | 8:00 AM – 4:30 PM

Full Time Permanent Position

Setting: PACE / Geriatric Patients

• Serve as the primary clinical triage and care navigation resource for participants and the interdisciplinary team.
• Provide centralized clinical assessment, symptom evaluation, monitoring, and care coordination support in an in-house, clinic-based setting (no home or community visits).
• Focus on timely clinical decision-making, escalation management, accurate documentation, and participant communication to support continuity of care and prevent unnecessary hospitalizations.
• Ensure compliance with CMS, state, and PACE regulatory requirements while promoting participant safety and quality outcomes.

• Current RN license in good standing
