Job Description

The Licensed Practical Nurse (LPN), Registered Nurse (RN), or Nurse Practitioner is responsible for the delivery of primary care to the clients of Gateway Homes. This position can be part-time or full-time. The target population are individuals with a major medical illness, and this position is responsible for delivering their medical care. This position will perform customary primary care exams and procedures.

Additionally, this position will be responsible for drawing lab work for the monitoring of physical and mental health.

Ancillary duties for this position include working with the staff on medication management, administering injectable medications, and potentially teaching a medication refresher course.

This position is Monday - Friday with holidays off.

Essential Duties and Responsibilities
  • Establishes a practice agreement with the facility medical director.
  • Works with mental health staff to identify those clients/residents requiring primary care services.
  • Performs intake and transfer physical examination for those clients/residents entering Gateway and those requiring a release physical.
  • Performs TB health screenings and/or PPD testing for facility staff.
  • Works with the facility pharmacy to ensure the client/resident medications and MARs are accurate.
  • Administers injections for clients/residents.
  • Performs complete and accurate health histories and medical record reviews.
  • Performs health assessments in the clinic setting.
  • Performs laboratory services and reviews laboratory data for clients/residents.
  • Collects comprehensive patient health data such as laboratory, radiological, and diagnostic studies to be used in the course of disease management.
  • Critically analyzes assessment data to construct appropriate differential diagnosis for further investigation, treatment, and/or referral.
  • Identifies expected outcomes derived from the assessment data and formulates appropriate medical and nursing diagnosis.
  • Develops a comprehensive plan of care with individualized expected outcomes appropriate to the patient’s needs and condition.
  • Initiates a therapeutic regimen to include education, interventions, treatments, medications, procedures and nutritional support within the scope of the appropriate practice agreement.
  • Initiates referrals to appropriate healthcare providers, facilities, agencies, and other community resources, including outpatient services, outpatient providers and referral to the hospital emergency room.
  • Initiates emergency/stabilizing procedures when indicated until emergency personnel can transport the patient to an appropriate facility.
  • Evaluates the patient's progress in attaining expected outcomes and provides updated ongoing recommendations.
  • Documents patient information in an accurate and timely manner according to organizational policies, procedures and guidelines.
  • Teaches the medication administration refresher course for staff licensed for medication administration.
  • Ongoing communication is to be maintained between this position and the collaborating physician. This may be done by direct consultation, phone contact, or secure email.
  • Evaluation of patient services provided and outcomes by this position will occur minimally on a quarterly basis. This may be achieved by joint rounds, patient care conferences, and/or chart reviews by the collaborating physicians.
  • Any other duties assigned by the Executive Director.
  • Attend any trainings deemed appropriate by the Executive Director.
Minimum Qualifications
  • Must be licensed by the Commonwealth of Virginia to practice as a Licensed Practical Nurse.
  • Must have a current CPR and TOVA certificate.
  • Ability to work independently and effectively with the clinical team.
  • Capacity to work with mentally ill adults and their families/representatives.
  • Appropriate computer skills.
  • Ability to work with outside agencies and other providers in triage of care for individuals.
  • Salary is negotiable depending on experience and credentials.
