Position SummaryOur Wellness Nurse (LPN) plays a vital role in ensuring the health and well‑being of residents. In collaboration with the Health and Wellness Director, the role provides high‑quality care through health assessments, medication management, and coordination with residents, families, and the care team.
Key Responsibilities- Perform resident assessments and reassessments, monitoring any changes in condition.
- Communicate with the Health and Wellness Director about resident needs to ensure consistent quality of services.
- Participate in hands‑on care, medication administration, and specific nursing tasks when necessary.
- Schedule and assign caregiving and med‑tech shifts to ensure adequate coverage and compliance with staffing guidelines.
- Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
- Assist in transcription of physicians’ orders and review of new physicians’ orders.
- Maintain accurate resident records, including care plans, incident reports, and medical documentation.
- Coordinate care planning with home‑health agencies on‑site, working within community and company policies.
- Address residents’ special needs, requests, and complaints.
- Coordinate admissions with the sales team, including supervising move‑ins.
- Understand community and state‑specific care regulations.
- Facilitate communication between residents, families, and healthcare providers to ensure seamless care delivery.
- Valid LPN/LVN nursing license required.
- High School Diploma or equivalent.
- Associate degree or higher in nursing preferred; additional coursework in management is an asset.
- Current First Aid and CPR license (or ability to obtain).
- At least four (4) years in geriatric nursing, including medication administration and pharmacy experience.
- At least one (1) year of supervisory experience (staff scheduling and management) preferred.
- Experience with memory care is a plus.
- Excellent attention to detail and integrity.
- Strong communication skills and teamwork mindset.
- Strong leadership and team management skills.
- Outstanding organizational skills.
- Positive attitude, empathy, patience, and commitment to treating residents with dignity and respect.
- Willingness to participate in on‑call rotation.
- Must meet all mandatory health requirements per state regulations.
