Position Overview

The Illinois Department of Public Health is seeking a highly motivated, detail oriented individual to serve as a field supervisor. This individual will oversee a multidisciplinary team that conducts surveys of long-term care facilities, including state licensure surveys, federal certification surveys and complaint surveys. This Field Supervisor is responsible for assigning licensure, certification and complaint surveys, reviewing written survey documentation, testifying in court and performing monitoring functions.

  • Under administrative direction, oversees Intermediate Care Facilities/Individuals with Intellectual Disabilities (ICF/IID) facility survey activities.
  • Serves as working supervisor.
  • Conducts state licensure, federal certification, and complaint surveys of ICF/IID facilities.
  • Develops ICF/IID facility survey schedules, reviews and processes survey packages and makes recommendations to Central Office.
