Responsible for creating, leading, and supporting assigned participants in their daily programming in the areas of academic, social, life and leisure skills as well as recreational activities. Assists in the development and implementation of Personal Centered Individual Service Plans (ISP) including social, community inclusion, communication, behavior, and skill development. Ensure compliance with all State and Federal regulatory guidelines, including Medicaid, Licensure, and CARF as well as other funding sources and company policy/procedure.
PrimaryDuties
- Provide active engagement and support for all individuals assigned, including community integration, training, leisure, communication, mobility, behavior, personal care (feeding, etc.), bus loading/unloading, medication, safety and risk management.
- Support planning and documentation for Person Centered Plans (PCP), ensuring accurate records and progress reporting.
- Maintain a safe, clean, and orderly environment; report unsafe conditions, medical concerns, and adhere to established protocols.
- Report incidents, allegations of abuse, neglect, and exploitation according to Service Source policy and procedures.
- Communicate with families, guardians, authorized representatives, residential providers, and Support Coordinators regarding participant illness, incidents, progress, and other matters.
- Perform related duties and responsibilities as assigned or requested.
- Maintain participant records, daily notes, quarterly reports per Medicaid, Licensure, and CARF standards.
- Adhere to health and safety regulations.
- Participate in PCP process and work with the team to implement, monitor, and review each individual’s plan.
- Utilize appropriate behavior intervention techniques and proper lifting, feeding, and transferring techniques.
- Complete and pass the Orientation to Development Disabilities (ODD) test and competencies.
- Participate in department, participant, and staff committee meetings.
- Attend required trainings as mandated by funding sources and the corporation.
- Required: High School diploma (or GED) plus one (1) year of experience working with adults with developmental disabilities or related field.
- Preferred: High School diploma (or GED) plus college-level courses in the human services field, or a bachelor’s degree in the human services field, and experience working with adults with developmental disabilities.

- Valid driver’s license and eligibility for Service Source’s driver’s insurance, if required; must pass online driving training.
- Pass a criminal background check and drug screening.
- Working knowledge of Microsoft Office and basic computer programs.
