Pay Range: $24.00- $28.50/hour (non-exempt), based on experience, qualifications, and location. Premium pay if working 3 out of 4 weekends per month!
What You'll Do As a Licensed Practical Nurse (LPN) Monitor Resident Health and Document Changes- Obtain necessary information to initiate and update the Plan of Care for each resident. Observe and document changes in resident’s physical, mental, or emotional status. Communicate resident changes with the interdisciplinary team and document all observations.
- Administer medications as prescribed or other physician-ordered treatments. Ensure accurate transcription of new physician orders and maintain clean, organized med carts and nursing stations.
- Educate residents about their medications, treatments, and side effects while respecting their right to make personal choices. Communicate changes in resident health to families, offering support and comfort during difficult times.
- Monitor and provide work direction to Nursing Assistants, offering guidance, instruction, and mentorship.

- Current Licensed Practical Nurse (LPN) license in the state of Minnesota.
- CPR certification required within 6 months of hire.
- Experience working with seniors in long-term care, home care, or assisted living settings is preferred.
- Ability to work effectively with a diverse population, including residents with varying values, backgrounds, and capabilities.
- Physical capability to perform job duties.
- Proficiency in reading, writing, and speaking English to communicate effectively with residents, families, and team members.
- Ability to pass state mandated background check.
