Hourly Rate

$15.50 – $18.00

Duties & Responsibilities
  • The Diversion Case Manager (DCM) works with youth referred to the program and determined to be eligible for Juvenile Justice Diversion services.
  • The DCM conducts a comprehensive assessment to develop Individualized Intervention Plans for youth.
  • Serves as the liaison between the Provider, the youth, their parents, OJJ, and other service professionals; schedules and facilitates Individual Intervention Plan meetings.
  • Guides the development of a mutually agreed-upon Individual Intervention Plan defining short- and long-term goals and orienting youth to the program; provides supportive counseling; maintains regular contact; monitors the youth’s needs and progress and updates the plan and records.
  • When community service is needed, ensures the activity reflects the youth’s skills, experience, aptitude and interests to address specific violations or behavioral concerns.
  • Serves as liaison to other community-based organizations to promote visibility of the program and ensure effective utilization of services to meet program outcome goals.
  • May assist in participant curfew checks and provides transportation as needed; complies with Eckerd’s Fleet Safety Program.
  • Maintains an average caseload of up to 25 youth.
Qualifications
  • Bachelor’s degree from an accredited college or university in criminal justice, social work, education, or an OJJ-approved related field, and a minimum of one year of professional experience working with at-risk or delinquent youth.
  • 5+ years of professional experience in social services or an education professional position working with at-risk or delinquent youth may substitute for a bachelor’s degree.
  • Must be able to meet requirements for Eckerd Connects Auto Insurance and be able to drive for business purposes.
  • Required to pass a drug screen in compliance with our Substance Abuse and Drug-Free Workplace policy.
