Family Service Coordinator

What Youll Do

• Coordinate accurate intakes, evaluations, and assessments to determine eligibility
• Facilitate and actively participate in the development and reviews of Individual Family Service Plans (IFSPs)
• Support families in accessing community resources beyond the child's condition
• Coordinate services and monitor delivery, including subcontractors and cross-agency providers
• Maintain up-to-date and accurate child files in compliance with accreditation and Medicaid standards
• Facilitate transition planning for children exiting the program
• Gather and manage a community resource register to better serve families
• Stay current through required training programs and professional development

• Education: Bachelors degree in Social Work, Psychology, Nursing, Special Education, Early Childhood Education, Counseling, or related field
• License: Valid drivers license required
• Background check and driving record check required
• Bilingual skills are a plus!

• Strong understanding of child and family services
• Ability to build relationships with families and community partners
