Why does the job exist?

Social workers provide psychosocial support to individuals, families, and vulnerable populations so they can cope with chronic, acute, or terminal illnesses, such as Alzheimer's disease, cancer, or AIDS. They also advise family caregivers, counsel patients, and help plan for patients' needs after discharge from hospitals. They may arrange for at-home services, such as meals-on-wheels or home care. Some work on interdisciplinary teams that evaluate certain kinds of patients, such as geriatric or organ transplant patients.

Some social workers help clients who face a disability, life-threatening disease, social problems, such as inadequate housing, unemployment, or substance abuse. Social workers also assist families that have serious domestic conflicts, sometimes involving children or spousal abuse. Additionally, they may conduct research, advocate for improved services, or become involved in planning or policy development.

How does it get done?

Will work to improve social and behavioral functioning of children while placed in the Receiving Center and supplement the e- case management services being provided by the primary worker as well as present case findings, and review case records; assist in arranging support services needed for the child/youth; maintain accurate records and progress notes; and assist with treatment and discharge planning and risk assessments when the child/youth is ready for discharge.

Will assist and support staff in performance-based outcomes measures by providing needed supportive services to youth placed at the Receiving Center by ensuring they: 35%
  • Provide 24/7 care to youth ages 11 years old and younger at the Receiving Center.
  • Complete the Admission and Discharge Log prior to a child or youth arriving.
  • Review and completes the Intake Packet with the child or youth and their primary PSD worker.
  • Review the RC Rules and Expectations and the NM Foster Child and Youth Bill of Rights with the child or youth .
  • Ensure to recommend additional services based on observations from direct care with clients.
  • Will also document activities and information in FACTS as required by agency policy and procedures to prepare: 35%
  • Prepare administrative reports
  • Case management notes documenting progress in placement
  • Behavior and interaction notes to provide feedback to primary worker and community partners working with youth
  • Maintain accurate records and progress surrounding treatment plans
  • Will also identify potential community-based services, share information to assist staff and conduct activities that promote safety, permanency and well-being. This employee will maintain a safe environment for all children and youth in the Receiving Center. If a child or youth is at risk of hurting themselves or others, the employee will provide the child or youth with one-on-one supervision as needed until cleared by a Behavioral Health Clinician.

    Ideal Candidate

    Experience as it relates to:
  • Working directly with Children, Youth, and their families;
  • Partner with Community Service providers as it relates to Children, and Youth;
  • Working with Youth in a school setting;
  • Determining eligibility for services that might be needed for the Children, Youth and Families;
  • Conduct and complete assessments of safety, risks, strengths, and family needs;
  • Developing Safety plans for the Children, Youth and Families;
  • Participate in multidisciplinary meetings;
  • Identify, provide, and/or arrange timely assessments/evaluations/ interviews;
  • Conduct medical case management activities for eligible clients.
  • Minimum Qualification

    Associate's Degree in Human Services or Social Sciences and two (2) years experience in mental health and/or substance abuse counseling. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling four (4) years may substitute for the required education and experience.

    Employment Requirements

    Must possess and maintain a valid Driver's License. Pre-employment background investigation is required and is conditional pending results.

    Working Conditions

    Position will perform work primarily in an office setting with extensive computer and phone usage and exposure to Visual/Video Display Terminal (VDT). Exposure to potentially violent hard-to-manage patients in volatile situations. Some travel may be required. May be required to work weekends, holidays and overtime.
