We provide services to all age groups. We support over 800 children annually through our Early Intervention Program and 1,500 adults in our Family Support Services. Experience
BA/BS preferred, or 2 years of related Developmental Disability (DD) experience.
Position Specific Essential Job Functions
Performs essential job functions with or without reasonable accommodation at an acceptable level.Initiate and maintain family and individual relationships to ensure Alta Mira Family Support Services (FSS) are delivered.Partner with families, as the experts on care for their family members with developmental disabilities, and ensure family participation in the training of direct care providers.Gather and communicate family input regarding service quality.Implement and monitor Family Support Services for those receiving services.Develop nurturing relationships with clients to facilitate desired outcomes and meet needs.Take initial referrals, schedule intake interviews, and complete them to gather information for planning service delivery.Develop individualized goals and objectives (Individualized Family Service Plan / IFSP or Individual Service Plan / ISP) in collaboration with clients, consulting with other Family Support Services staff and/or agencies as appropriate.Schedule and conduct regular home visits and/or observation visits to review goals, service quality, health, and safety needs.Respond to requests from families and/or individuals for changes in program or service plans.Provide ongoing Family Support Service coordination for clients.Follow Service Coordination policies and procedures as outlined by funding and regulatory agencies, including Alta Mira Specialized Family Services.Serve as a resource for clients and other community providers, sharing policy and procedural information.Assist clients in locating and accessing needed services within Alta Mira or through other providers.Deliver and monitor services to ensure they are provided in natural environments and integrated into the individual’s daily routine.Collaborate with families, program participants, and community resources to promote independence.Develop, plan, lead, and participate in educational, training, and outreach activities, including planning and delivering lectures or workshops as requested. Share pertinent information with supervisors, team members, the agency’s services and/or programs.Participate in the development and implementation of Family Support Services training and orientation programs.
Training & Orientation Responsibilities
