Job Purpose
The case manager is responsible for using sound professional judgment and best practices to ensure quality case management services are provided to the service population, especially in crisis situations, while maintaining compliance with established state and federal standards. The manager monitors the assigned caseload of clients participating in a short‑term life‑management program; establishes program plans and goals; evaluates client progress by conducting routine meetings;
and serves as an advocate to acquire services that enable clients to functionally cope with their environment.
The case manager is responsible for intake, coordinated assessment, individualizing housing plans, making appropriate community referrals, maintaining contact with the client, and ensuring the timely completion of individualized housing plans.
EssentialJob Responsibilities
- Coordinate referrals, service planning, and documentation of services for the assigned caseload.
- Interview applicants to determine eligibility for program enrollment based on established guidelines; conduct needs assessments, obtain pertinent information, and establish program goals.
- Develop client intake and case plans; instruct clients in completing necessary paperwork; restore release‑of‑information forms; complete social histories each week.
- Meet regularly with clients to discuss and evaluate progression; create accurate and up‑to‑date records; monitor client behavior, group attendance, medication use, and incident reports.
- Plan, coordinate, and facilitate life‑management classes to help clients develop coping skills; secure speakers and workshop presenters as needed.
- Act as mentor, providing positive examples of hygiene, appearance, record retention, budgeting, and scheduling.
- Refer clients to programs or agencies based on needs and program plans.
- Complete discharge plans for exiting clients; transfer file contents to non‑active files.
- Meet deadlines set by program supervisor and partners.
- Meet federal and state regulatory guidelines applicable to the position.
- Maintain a caseload in accordance with agency policies, procedures, and contract standards.
- Participate in workshops, seminars, and other professional development activities.
- Maintain required weekly client contacts.
- Engage clients in the casework process.
- Assess changes in behavior, circumstances, or conditions that may affect client safety.
- Compile, prepare, submit, and maintain accurate records, files, forms, statistics, and other required information per agency policies.
- Participate in weekly case staffing meetings.
- Maintain confidentiality regarding service population and program operations.
- Demonstrate exceptional customer service by placing the client first in all interactions.
Education: Bachelor’s degree in Social Work, Psychology, Criminal Justice, Sociology, or related field.
Experience: 1+ year related experience or training, including work in crisis intervention.
Language: Proficiency in Spanish and English (written and spoken).
RequirementsAttendance: Must maintain regular and acceptable attendance as determined by the employer.
Licenses: Driver’s license with a clean record.
Vehicle: Daily use of a vehicle without prior notice; must travel to various locations as business demands.
Other: Must not pose a direct threat or significant risk to the safety or health of self or others.
