Tender Love Community Center is seeking a compassionate, organized, and mission-driven Rapid Rehousing Case Manager to join our team under the Continuum of Care (CoC) Program. This individual will provide trauma-informed, client-centered case management to individuals and families enrolled in our Rapid Rehousing (RRH) program. The Case Manager will report directly to the CoC Program Director and play a critical role in helping clients transition from homelessness to stable, independent living.

Compensation: Commensurate with experience

Key Responsibilities:

  • Provide direct case management services to RRH clients, including individualized housing plans, needs assessments, and ongoing support.
  • Assist clients in locating, securing, and maintaining safe and affordable housing.
  • Collaborate with landlords, housing providers, and community resources to identify appropriate housing opportunities.
  • Conduct home visits, regular follow-ups, and reassessments to ensure client progress and program compliance.
  • Coordinate supportive services, including employment support, mental health referrals, and life skills development.
  • Ensure accurate and timely documentation in HMIS and internal databases.
  • Support clients in achieving long-term housing stability and self-sufficiency.
  • Attend regular team meetings and training to remain up to date with CoC and HUD program requirements.

Qualifications:

  • Bachelor’s degree in social work, Human Services, or a related field (or equivalent experience).
  • At least 1 year of case management or related experience, preferably with homeless or vulnerable populations.
  • Familiarity with HUD’s Rapid Rehousing model and trauma-informed care practices.
  • Strong organizational, communication, and documentation skills.
  • Valid driver’s license and reliable transportation for home visits.
