Position: NP / PA PRIMARY CARE - NORTHEAST HEIGHTS CLINIC

The UNM Hospitals Northeast Heights Clinic is looking for a motivated clinician who is interested in joining our team to help care for patients in our community. The clinic is located in the Northeast area of Albuquerque. The Internal Medicine practice focuses on adult patients with complex acute and chronic medical issues. Working together with a multi-disciplinary team consisting of MDs, Residents, PAs, NPs, Psychologists, Clinical Pharmacists, Care Managers, Social Workers, and Community Health Workers to help provide an overall comprehensive care approach.

With our Patient Centered Medicine Home (PCMH) clinic model, providers collaborate with families to identify and satisfy the full range of patient needs and preferences. Advanced Practice Providers (APPs) work semi-autonomously with collegial support that is readily available. Experience is preferred with option to train a highly motivated new graduate.

Position Summary:

Function in a primary care setting in the role of practitioner, mentor, consultant, educator, researcher and manager. Establish goals and priorities in accordance with patient and staff needs and organizational objectives. Ensure adherence to Hospitals and departmental policies and procedures. Patient care - Women's Specialties:
Ages of patients served restricted to adolescent, adult and geriatric age groups. Family Practice Clinic:
Ages of patients served may include neonate, pediatric, adolescent, adult and geriatric age groups. Senior Health Clinic:
Ages of patients served restricted to geriatric age groups.

Detailed responsibilities:

  • PATIENT CENTERED MED - Adhere to and promote the core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice as applicable
  • PATIENT CARE - Assess, diagnose and develop treatment plans; implement treatment, follow-up and evaluate patient within the scope of practice protocols as outlined by state statute
  • PATIENT CARE - Evaluate and revise intervention care/treatment plan with patient and family as necessary to achieve improved health
  • PATIENT CARE - Provide education for patients and/or families about preventative care, medical issues and use of the prescribed medical treatment
  • DOCUMENTATION - Provide accurate documentation of patient care, document orders, directives, procedures and patient responses in the medical record and communicate them to other members of the health care team; maintain accurate, legible and confidential records
  • PATIENT CARE - Perform invasive and non-invasive diagnostic and therapeutic procedures
  • ROLE MODEL - Act as a role model through conduct, appearance, communication, mutual respect, leadership, ethical decision-making, critical thinking, and problem solving skills
  • PATIENT CARE - Work as a team member with medical/nursing/counseling staff to provide comprehensive care and promote continuity of care
  • EDUCATION - Guide, teach, support and serve as a clinical resource nursing and medical staff
  • RESEARCH - Support research by conducting or participating in research studies; incorporate findings in clinical practice
  • DEVELOPMENT - Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops
  • PATIENT SAFETY 1 - Follow patient safety-related policies, procedures and protocols
  • PATIENT SAFETY 2 - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes
