Overview

Sub‑Acute, Long‑Term Care is a sector of healthcare exploding with potential and work‑life balance flexibility (no on call or weekends).

We are seeking a Full Time (40 hours per week) provider for our Uptown Rehabilitation Center located in Albuquerque, NM. The position can be filled by a nurse practitioner or physician assistant. New grads are welcome to apply.

The base salary range for this position is $115,000 – $125,000 per year, in addition to our no‑cap incentive compensation productivity bonus.

Responsibilities

The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. The provider collaborates on‑site with the Center Leadership Team for center‑specific goals and priorities, and works with the Medical Director and/or Attending Physician for patient‑specific medical care.

The provider delivers primary and urgent care services to nursing center residents in collaboration with other medical providers in the center. They work collaboratively with Center Leadership to achieve Align Med Partners’ mission of improving the lives we touch through the delivery of high‑quality health care and everyday compassion.

Qualifications

The Advanced Practice Provider must be a graduate from an accredited School of Nursing program, licensed as a registered nurse, certified as a Nurse Practitioner by completing an accredited Nurse Practitioner Program with a Master's Degree in Nursing, and have a concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP). Other concentrations/specialty tracks may be considered based on experience and business need.

The Physician Assistant must be a graduate of a program accredited by the ARC‑PA and hold the PA National Certifying Exam (PANCE) to use the PAC designation. PA state licensure is required, along with NCCPA certification.

A minimum of two years of experience working with adult or geriatric populations is preferred. In states where practice is allowed, the APRN must hold an unrestricted professional license and DEA for each state in which they practice. PAs must similarly have an unrestricted professional license and DEA for each state in which they practice.

Restrictions apply based on collective bargaining agreements, applicable state law and factors such as pay classification, job grade, location, and length of service. Eligibility for full‑time benefits requires a minimum of 30 hours per week. Posted Salary Range: USD $115,000 – $125,000 per year.
