The Inpatient Gynecologic Oncology team at UNMH is seeking a highly motivated and compassionate Advanced Practice Provider (APP) to join a dynamic, multidisciplinary care team. This role involves direct collaboration with physicians, fellows, residents, nurses, social workers, and other healthcare professionals to provide comprehensive care for hospitalized patients undergoing oncologic treatment. The APP will manage high-acuity patients through clinical oversight, care coordination, patient and family education, and discharge planning, contributing to a patient-centered and team-based approach to care.
Responsibilities- Manage daily inpatient census and consults, including ED evaluations and hospice transitions
- Coordinate care for patients with multiple comorbidities in collaboration with consulting services
- Lead discharge planning, post-op education, and follow-up to ensure safe transitions and reduce readmissions
- Provide patient and family education, emotional support, and lead goals-of-care discussions
- Facilitate transfers to SNFs and rehab facilities, ensuring insurance and documentation compliance
- Participate in daily rounds and serve as a clinical resource for the care team
- Supervise and teach residents, fellows, and medical students
- Assist in OR, coordinate chemotherapy initiation, and provide treatment education

Position SummaryFunction in an inpatient / urgent care area of advance practice in the role of practitioner, mentor, consultant, educator, researcher and manager. Establish goals and priorities in accordance with patient and staff needs and organizational objectives. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult and geriatric age groups.
Detailed Responsibilities- PATIENT CARE - Assess, diagnose and develop treatment plans; implement treatment, follow-up and evaluate patient within the scope of practice protocols as outlined by state statute
- PATIENT CARE - Perform patient history and physical exams as needed for procedures or admissions; document findings noting pertinent normal and abnormal findings
- PATIENT CARE - Complete admitting orders, daily rounds and orders, consultations with nurses, specialist and ancillary staff, discharge summary, transfers, medication management and other orders as required for the management of patient care
- PATIENT CARE - Perform invasive and non-invasive diagnostic and therapeutic procedures
- PATIENT CARE - Order and interpret appropriate diagnostic procedures and tests. Explain the necessity of preparation, nature, and anticipated effects of procedures to patient and patient’s family
- PATIENT CARE - Evaluate and revise intervention care/treatment plan with patient and family as necessary to achieve improved health
- PATIENT CARE - Provide education for patients and/or families about preventative care, medical issues and use of the prescribed medical treatment
- DOCUMENTATION - Provide accurate documentation of patient care, document orders, directives, procedures and patient responses in the medical record and communicate them to other members of the health care team; maintain accurate, legible and confidential records
- PATIENT CARE - Work as a team member with medical/nursing/counseling staff to provide comprehensive care and promote continuity of care
- EDUCATION - Guide, teach, support and serve as a clinical resource nursing and medical staff
- TRAINING - Act as a role model for advanced practice professions, precept nurses, other advanced practice nurses and physician assistant students
- ROLE MODEL - Act as a role model through conduct, appearance, communication, mutual respect, leadership, ethical decision-making, critical thinking, and problem solving skills
- RESEARCH - Support research by conducting or participating in research studies; incorporate findings in clinical practice
