We are seeking a skilled and compassionate Weekend On-Call & Admissions Registered Nurse (RN) to join our hospice team. This position plays a vital role in responding to urgent patient needs after hours and welcoming new patients and families into our care.
Key Responsibilities- Respond promptly to on-call patient needs, providing compassionate clinical support and interventions.
- Conduct timely and thorough admissions for new patients entering hospice services.
- Assess patient conditions, manage symptoms, and coordinate care with physicians as needed.
- Provide education, emotional support, and guidance to patients and families.
- Document all visits, assessments, and care plans accurately and in compliance with hospice policies.
- Communicate effectively with team members to ensure continuity of care.
- Graduate of an accredited school of nursing.
- Current and valid RN license in the State of New Mexico.
- Minimum of 1 year of clinical nursing experience.
- Strong assessment, symptom management, and communication skills.
- Valid driver’s license and current automobile liability insurance.
- Experience in hospice, home health, or palliative care.
- Admissions experience and comfort with electronic medical records.
