OverviewPresbyterian is seeking an LPN to provide direct patient care to an assigned group of patients in the home setting. Common responsibilities include specimen collection, vital signs, assist with video calls, medical administration per LPN scope, wound care, foley care, and EKGs, and engaging in effective communication with the interdisciplinary team to ensure coordinated, patient-centered care.

- Minimum new graduate or experienced nurse from an accredited practical nursing program.
- Licensed Practical Nurse (LPN) in NM.
- High School Diploma or GED.
- BLS certification.
- Adheres to the LPN scope of practice by contributing to the assessment of the health status of individuals, families, and communities.
- Delivers direct patient care, including administering treatments as prescribed, assisting nurses and physicians with various procedures, and documenting individual patient needs, actions taken, and patient responses.
- Administers prescribed medications, including medications given through established parenteral/intravenous infusions as prescribed by physician or standard protocols; documents according to policy.
- Documents and reports patient condition, care delivery and post‑procedure follow‑up; assesses changes in patient condition and reports to nurse and physician as needed.
- Incorporates nursing activities that create a compassionate, supportive and therapeutic environment for patients and staff to promote comfort and healing.
- Applies Exceptional Patient Experience principles and incorporates them into care interactions.
