Job Overview
Giving Home Health Care is a leading provider of personalized in‑home care services across Arizona, Colorado, Kentucky, Missouri, Nevada, New Mexico, Tennessee, Texas, and Utah. We support individuals impacted by health conditions related to their work in nuclear facilities for the Department of Energy.
PayBase pay range: $80,000.00/yr – $/yr. Actual pay is based on skills and experience.
Responsibilities- Review Plans of Care (POCs), letters of medical necessity (LMNs), and clinical documentation for accuracy and compliance.
- Review and sign monthly case management (CM) notes.
- Perform regular spot checks of nurse charting to ensure accuracy, compliance, and consistency.
- Complete supervisory visits for patients without an assigned nurse and for patients assigned to an LPN.
- Complete T1001 for patients without an assigned nurse and for patients assigned to an LPN, including mid‑auth increases or 50% of patient census.
- Collaborate with Care Team Directors and DON regarding patient clinical needs, staffing alignment, and escalation of red‑flag cases.
- Notify physicians and interdisciplinary team of patient change in condition.
- Serve as a clinical resource for Care Team Directors and Patient Coordinators to resolve complex cases.
- Support Directors in reviewing nurse and HHA utilization to ensure proper allocation of hours.
- Meet with patient/family regarding utilization of care plan.
- Participate in patient/family meetings when escalations require clinical involvement.
- Active Registered Nurse (RN) license in the state of practice (BSN preferred).
- 3+ years nursing experience; home health, case management, or clinical leadership experience strongly preferred.
- Knowledge of DOL/EEOICPA guidelines and home health regulations a plus.
- Strong organizational and critical thinking skills.
- Ability to provide mentorship and clinical education to interdisciplinary staff.
- Proficient in EMR systems; experience with IGEA preferred.
