Role Overview

Nurse Practitioners see patients independently, collaborate with physicians, and build meaningful relationships to foster trust and understanding of care plans.

Responsibilities

Provide exceptional primary care and conduct office visits for routine and acute issues.

Administer wellness visits and health risk assessments with thorough documentation.

Coordinate care with other providers and specialists.

Lead in population health initiatives, ensuring preventive screenings and health assessments.

Assist with phone triage and patient outreach.

Educate patients on their health conditions and treatments.

Participate in promotional activities and conduct home visits as needed.

Perform other duties as assigned.

Qualifications

Master's degree in Nursing, DNP, or NP Certification.
Certification in Family, Adult-Gerontology, or Gerontological Nurse Practice.
Active, non-probationary NP license and DEA registration.
US Work Authorization.

Preferred Skills

Experience in primary care, geriatrics, or family medicine.
Knowledge of HCC documentation, ICD-10 coding, and HRAs.
Team-oriented with a passion for population health and bilingual abilities.

Additional Information

Work hours:
