Collaborate with families, service providers, and community partners to develop and implement individualized, strengths-based care plans. Build collaborative and trusting relationships with families, empowering them to actively participate in their care planning and decision making processes. Guide families through the four phases of the HFW process.
Additional Areas of Responsibility:
Facilitate regular Child and Family Team (CFT) meetings, ensuring that all voices are heard and respected. Maintain accurate and timely documentation of all services, progress, and outcomes in compliance with organizational and regulatory standards. Coordinate with community resources, service providers, and support systems to ensure comprehensive and holistic services.
Other Information/Requirements:
Bachelor’s degree in Social Work, Psychology, or a related field with a minimum of two years lived and/or paid experience working with the target population;
High School Diploma or GED with a minimum of six years lived and/or paid experience working with the target population. Understanding of the HFW model
