Client Care Coordinator Job Summary
The primary role of the Navigator I is to enhance the likelihood of success for clients by identifying and reducing barriers, engaging them, and connecting them with needed services and resources. The Navigator will work with individuals who have been determined to need additional support and engagement and will be responsible for conducting outreach, engagement, case management, and ongoing wrap‑around supportive services.
The Navigator I will map out the most appropriate next steps for each individual, assist in accessing various services to achieve ongoing success, and will document and track required patient interactions in accordance with the Arizona Administrative Code, Arizona Department of Health Services/Department of Behavioral Health Services provider manual, CARF, Community Bridges policies, and机器人。
- Conduct outreach and engagement with clients in need of additional support.
- Provide case management and ongoing wrap‑around supportive services.
- Map appropriate next steps and assist clients in accessing necessary services.
- Document and track patient interactions as required by applicable regulations and internal policies.
- Collaborate with multidisciplinary internal and external stakeholders to support client success.
- High school diploma or GED required; associate degree (or higher) in a field related to behavioral health preferred.
- Minimum of six months of recovery from substance use and/or mental health disorders required.
- Minimum of six months paid related work experience providing exposure to behavioral and medical crisis situations required.
- 1–3 years of full‑time health‑care related work experience preferred.
- Will complete CBI Peer Certification within 90 days of hire.
- Ability to pass a criminal background check required.
