OverviewNow hiring a Care Coordinator III – Santa Fe, NM
. Facilitates a team approach, including the Interdisciplinary Care Plan team, to ensure appropriate interventions, cost‑effective delivery of quality care and services across the continuum. Collaborates with team members—caregivers, legal representatives, physicians, care providers, and ancillary support services—to address care issues, member needs, and disease processes (medical, behavioral, social, community‑based, or long‑term care services). Coordinates care of individual clients using assessment, care planning, implementation, coordination, monitoring, and evaluation for cost‑effective and quality outcomes.

- Job is based at Rev Hugh Cooper Admin Center.
- Work hours:
Monday‑Friday, weekday schedule.
- Experience in utilization management, quality assurance, home care, community health, long‑term care, or occupational health; CCM certification preferred or required within 3 years of hire.
- Proficiency in Microsoft Word, Excel, and Outlook; experience analyzing trends from decision support systems.
- Master’s degree with 2 years experience, bachelor’s degree with 4 years, associate’s degree with 5 years, or 10 years of experience in lieu of other education requirements.
- Supports health plan members.
- Facilitates a team approach to ensure appropriate interventions and cost‑effective delivery of quality care across the continuum.
- Collaborates with interdisciplinary team members to address member needs and disease processes across medical, behavioral, social, community‑based, and long‑term care services.
- Provides care coordination for members with chronic conditions, conducts comprehensive risk assessments, develops plans for authorization of services, and serves as point of contact during transitions.
- Assesses and reviews plans of care regularly to identify gaps, trends, and improvements; gathers clinical path variance data.
- Implements, coordinates, and monitors strategies to improve health and quality of life outcomes; advocates for member needs and evaluates intervention effectiveness.
- Provides assistance to members regarding care, providers, or delivery system.
- Conducts face‑to‑face home visits as required.
- Educates providers, support staff, members, and families on care coordination and health strategies.
- Maintains professional relationships with external stakeholders and promotes appropriate use of clinical and financial resources.
- Generates reports aligned with care coordination goals.
- Complies with Case Management Society of America Standards and CCMC Code of Professional Conduct.
- Participates in Interdisciplinary Care Team (ICPT) meetings.

Maximum offer for this position is up to USD $45.82/hr.
The compensation range for this role considers experience, training, internal equity, and other business and organizational needs.
