CASE MANAGER SOCIAL WORKER - SUPERVISOR
Supervise clinical therapy, social work, and case manager social work staff. Responsible for initial review, triage and assignment of patients to case managers and for coordination of all systems and services required for a structured, multidisciplinary, patient‑centered care team approach. Provide leadership to department staff, serve as a resource and role model, act as the primary contact for patients, families, healthcare team members, and community resources.
Ensure adherence to hospital and departmental policies and procedures. Patient care assignments may include neonatal, pediatric, adolescent, adult, and geriatric age groups.

Compensation: Minimum $33.07/hr;
Maximum $41.77/hr compensation is based on experience, education, and other considerations.
- Patient‑centered medical – adhere to core expectations of the Patient Centered Medical Home or Specialty Practice.
- Assignment – oversee and assign case loads and staff hours.
- Leadership – provide leadership through identification of problems, opportunities for improvement, program planning, implementation, and evaluation.
- Performance – assist in developing standards of performance, evaluating performance, and recommending personnel actions.
- Plan of care – develop comprehensive multidisciplinary plan of care.
- Data – perform assessment, collect and analyze information in collaboration with patient, family, and healthcare team.
- Assessment – assess patient’s clinical, psychosocial status and current treatment plans.
- Needs – assess patient/family needs and provide treatment options, resources, and discharge planning.
- Collaboration – develop collaborative relationships with other departments, services, and community agencies.
- Discharge planning – conduct timely discharge planning.
- Interventions – monitor and evaluate patient responses to interventions.
- Variances – review variance patterns from protocols and implement resolution strategies.
- Education – ensure instruction to patient and family based on identified learning needs.
- Information – assist in developing activities to disseminate information to health care team.
- Continuity of care – collaborate to ensure continuity throughout settings.
- Related work – perform related duties as required.
- Administration – monitor expenditures, prepare reports, participate in research, propose policy changes.
- Supervision – develop efficient structure, supervise employees, perform appraisals, counsel, schedule work.
- Development – enhance professional growth through education and workshops.
- Staff – assist in interviewing, hiring, orienting, training, coaching, evaluating staff.
- Patient safety – follow and improve patient safety policies and procedures.
Education: Master’s Degree
Specialization: Related discipline
Experience: 4 years directly related experience
Credentials: LMSW or LPC (essential); CCM, ACMA‑SW, C‑ASWCM (if prior to 07/2017) (non‑essential)
Physical Conditions: Sedentary work; may require lifting up to 10 lbs, occasional walking or standing. May travel to work sites and perform on‑call duties, rotating shifts, holidays, and weekends.
Languages: Bilingual in English and Keres, Tewa, Tiwa, Towa, Zuni, or Navajo (non‑essential)
