Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services throughout Arizona to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.
Job SummaryA Lead Navigator is an individual in recovery from AOD use and/or GMH (≥1 year) with at least 90 days experience as a Navigator or Peer Support Specialist. The Lead Navigator works with homeless individuals, providing outreach and supportive services to help them end homelessness. Responsibilities include outreach, engagement, case management, and providing wrap‑around supportive services. The Lead Navigator mentors newer Navigators, coordinates and monitors community‑based services, and works with multidisciplinary team members including nurses, counselors, and clinical leads.
Skills& Requirements
- High school diploma or GED required. Associates Degree (or higher) in behavioral health preferred.
- 1-3 years of full‑time health‑care related work experience preferred.
- Equivalency of education and experience acceptable.
- Valid driver’s license.
- Clean 39‑month motor‑vehicle record.
- Behavioral Health Technician (BHT) certification in accordance with A.A.C. R9‑10‑101.33 and CBI policies.
