• Perform spiritual assessments and risk assessments of complicating bereavement factors.
• Attend interdisciplinary team meetings and collaborate with team members to include spiritual needs in the plan of care.
• Refer to or coordinate with local clergy as requested by the client and family.
• Coordinate care with other professionals to meet the emotional and psychological needs of the client and family.
• Communicate effectively and respectfully with a diverse client population and co-workers.
• Assist families with funeral and memorial services when indicated.
• Help lead bereavement services for survivors.
• Submit required documentation in a timely manner.

• Preferred training includes a bachelor’s degree in religious studies or a related subject and a master’s degree in theology, divinity, or pastoral studies; hospice chaplain certification is also a plus.
• Current CPR with BLS card.
• Valid driver’s license, at least state minimum auto insurance, and an operational vehicle.
• Good understanding of Federal, State, and local laws and regulatory guidelines governing hospice operations.
• Great organization skills and an ability to work with minimal supervision.
• Ability to remain calm in emergency or crisis situations and to be close to clients who may be actively dying.
• Commitment to adhere to professional boundaries.
• Computer/Technology literacy and experience documenting on an Electronic Medical Record (EMR) system.

• Duties require extensive standing, walking, and sitting.
• Requires lifting, positioning, pushing, and/or assistance with transferring patients.
• Requires frequent reaching, stooping, bending, kneeling, and crouching.
