Position Summary

Coordinate and provide services and resources to individuals and families to promote rehabilitation, recovery, and resiliency. Identify strengths that aid recovery and address barriers to independent functioning in the community. Support individuals and families in crisis situations with personalized interventions to help them make informed, independent decisions. Patient care may include children at risk of or experiencing serious emotional, neurobiological, and/or behavioral disorders;

adults with severe mental illness; individuals with chronic substance abuse; and/or individuals with co-occurring disorders. Ensure adherence to hospital and departmental policies and procedures. Patient care assignments may include neonate, pediatric, adolescent, adult, and geriatric age groups.

Responsibilities
  • PATIENT SAFETY 1 - Follow patient safety-related policies, procedures and protocols
  • PATIENT SAFETY 2 - Demonstrate proactive approach to patient safety by seeking opportunities to improve policies and processes
  • PATIENT SAFETY 3 - Identify and report environmental conditions or situations that may put a patient at undue risk
  • PATIENT SAFETY 4 - Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner
  • PATIENT SAFETY 5 - Encourage patients to participate in their own care by asking questions and reporting unclear or concerning situations
  • PATIENT CENTERED MED - Adhere to core expectations of Patient Centered Medical Home or Patient Centered Specialty Practice as applicable
  • NAVIGATION/PATIENT ADVOCACY - Remove barriers to access culturally competent medical services; assist patients in accessing community resources and financial assistance programs
  • COLLABORATION - Work with medical providers, multidisciplinary team members, and community resources to meet treatment goals
  • LIFE SKILLS - Facilitate development of daily living skills, coping, socialization, and community functioning; support adaptation to home, school, work, and other environments
  • DEVELOPMENT - Plan for professional growth based on self-assessment, evaluation, and feedback
  • WELLNESS - Provide information on mental health conditions, mentor recovery and resiliency, teach symptom monitoring and relapse prevention
  • WELLNESS - Discuss medications with patients; support medication regimen adherence and related self-motivation for independent living
  • SERVICE PLANS - Assist in developing and coordinating service plans including recovery/resiliency management and crisis management plans; discuss advance directives when requested
  • RESOURCE COORD - Help clients access rehabilitative, medical, and other services; assist with financial/medical benefits and housing; support behavioral health consumers involved in the judicial system
  • SUPPORT SERVICES - Encourage development of natural supports in workplace, school, family, and community
  • ASSESSMENT - Identify strengths, needs, and barriers; conduct ongoing assessments to ensure services meet needs
  • CRISIS INTERVENTION - Assess and intervene in crisis, develop crisis management plans, and promote alternatives to hospital emergency services
  • TRACKING - Coordinate and monitor service use, track client activities related to the care plan, and maintain contact with providers
  • DOCUMENTATION - Document client encounters and billing; maintain current client files
  • STATISTICS - Maintain and report program statistics
  • DEVELOPMENT - Engage in ongoing professional development
Qualifications
  • Education - Essential:
    High School or GED
  • Experience - Essential: 1 year directly related experience;
    Nonessential:
    Bilingual English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo
  • Credentials - Essential: CPR for Healthcare/BLS, Certified Peer Specialist recognized in NM, Valid New Mexico Driver's License, UNM Vehicle Operator's Permit within 60 days
  • Physical Conditions - Light work; up to 20 lbs occasionally, up to 10 lbs frequently; may require walking/standing; may involve production pace
  • Working conditions - Essential:
    Exposure to dirt, dust, fumes, odors, bad weather, noise; subject to random alcohol and substance testing
