Physical Therapist Assistant, Home Health

Join to apply for the Physical Therapist Assistant, Home Health role at Center Well Home Health. As a therapist, you’ll help patients regain strength, mobility, and independence from their homes. Personalized care focuses on rehabilitation and functional improvement to empower patients to perform everyday activities and improve quality of life.

Responsibilities
  • Administer the physical therapy treatment plan developed by the Physical Therapist.
  • Administer non‑complex active and passive manual therapeutic exercises, massage, traction, heat, light, cold, water, and electrical modalities.
  • Accurately document daily, weekly, or monthly client records and submit clinical paperwork to the office on time.
  • Attend educational meetings and coordinate departmental in‑services.
  • Notify the PT or Executive Director of equipment failures, lack of supplies, staff or client changes.
  • Schedule clients and perform varied office tasks.
  • Apply infection control and universal precautions in patient care activities.
  • Accept clinical assignments that align with education and competence to care for patients.
  • Promote compliance with payor requirements through education, coaching, and assistance.
Required Experience & Skills
  • At least two years of experience as a Physical Therapy Assistant.
  • Associate degree in Physical Therapy from an accredited program.
  • Current state license to practice Physical Therapy.
  • Valid driver’s license, auto insurance, and reliable transportation.
  • Proof of current CPR certification.
  • Comprehensive knowledge of geriatric population changes.
  • Self‑directed, enthusiastic, and open to constructive feedback.
  • Good verbal and written communication and organizational skills.
  • Professional image and attitude toward patients, families, and coworkers.
  • Meeting continuing education requirements of Center Well Home Health and licensing board; maintaining clinical competency.
  • Flexible schedule and willingness to travel locally.
  • Ability to maintain confidentiality.
  • Fluency in English (reading, writing, speaking).
  • Good attendance record.
Pay and Compensation

Base pay: $58,400 – $80,000 per year. Pay per visit/unit: $37.00 – $52.00.
