Position SummaryUnder the close supervision of the Health Center Manager and in close partnership with the Dental Clinical Supervisor, the Dental Patient Care Facilitator assists patients with diverse needs, including an understanding of dental insurance, benefits, deductibles, treatment plans, and the urgency of care. The role requires explaining all information in layman’s terms, tracking and following up with dentists and the Dental Billing Department, scheduling appointments, confirming appointments, and conducting post‑visit follow‑up calls.
The facilitator supports patients in navigating other services offered by FCCH, aids in community resources, and helps coordinate referrals to specialists and other medical departments.
- Coordinate insurance benefits, eligibility, frequency limitations, procedure coverage, deductibles, and patient financial responsibilities for each treatment, and articulate this information clearly to patients.
- Coordinate, track, and follow up all outgoing medical and behavioral health referrals.
- Manage and schedule all incoming referrals from medical to dental services.
- Track biopsy lab orders and assure patient notification of results, facilitating referrals as needed.
- Audit charts to ensure sealants are properly treatment planned, scheduled, and completed.
- Support effective tracking of quality measures.
- Make appointment confirmation calls and postoperative procedure calls.
- Provide detailed treatment plan consultation in person or over the phone.
- Participate in community outreach and health fairs.
- Maintain social media presence for the dental site.
- Work the ASAP list daily to fill gaps from last‑minute cancellations across multiple providers.
- Conduct telehealth patient contact to support provider‑recommended behavior changes.
- Develop patient education materials.
- Support patient access to community resources affecting social determinants of health.

- Minimum
Education and Experience:
High School graduate or GED certificate; 2 years experience working as a Dental Assistant. - Bi‑lingual English/Spanish strongly preferred.
- Licenses/Certifications: CDHC license preferred.
- Knowledge, Skills, and Abilities:
- Knowledge of patient dental insurance and dental procedures and costs.
- Demonstrated knowledge of applicable FCCH patient‑related forms and programs.
- Excellent communication skills over the telephone and in person.
- Ability to educate and motivate patients toward behavior change.
- Knowledge of planning and scheduling processes.
- Understanding of pre‑certification requirements, procedures, and documentation of third‑party medical insurance payors.
- Problem‑solving and analytical abilities.
- Proficient with personal computers and related software applications.
- Familiarity with compliance programs, laws, and regulations.
- Maintains quality, safety, and infection control standards.
- Basic general office procedure knowledge (filing, copying, faxing).
- Multi‑line telephone usage to schedule appointments.
- Computer use for patient data entry, report generation, and electronic receipts.
- Takes initiative to resolve patient concerns and problems.
- Works well with diverse groups of people.

- Good dexterity to operate personal computer and office equipment; occasional lifting and carrying related to office duties.
- Must be able to read a computer monitor and output accurately; must communicate clearly and accurately for work and safety compliance.
- Work primarily inside a controlled office environment; normal safety precautions required.
- Potential remote work; travel around the Albuquerque metropolitan area may be required.
- Work schedule:
Monday–Friday, with weekends possible to meet deadlines.
