RN - Hospice

This is a 13-week contract position.

Days ) with rotating weekends.

The Hospice teams support patients and family members through the final stages of their illness. Services are offered in various home settings, including the patient’s own residence, skilled nursing facilities, assisted living facilities, and adult foster homes. The role involves providing intermittent skilled nursing visits, pain and symptom management, medical social services, counseling, spiritual support, and bereavement support.

• Deliver skilled nursing care and support to hospice patients.
• Manage pain and symptoms effectively.
• Collaborate with a multidisciplinary team to provide comprehensive hospice services.
• Educate patients and families on end-of-life care and resources.
• Maintain thorough documentation of patient care and communication.

• Current unencumbered Oregon RN Licensure is required.
• Healthcare Provider Level BLS certification is required.
• BSN and/or previous home health/hospice or case management experience preferred.
• Broad-based nursing experience to ensure specialized skills needed for hospice nursing is preferred.
• A valid driver's license and auto liability/property damage insurance are required.
