- Core responsibilities: conduct comprehensive psychosocial assessments for patients and families; lead discharge planning and post‑discharge care coordination; connect patients with housing, transportation, financial aid, language services, and community resources; provide crisis intervention and safety planning as needed; advocate for patient preferences and equitable access to services; document care plans, track outcomes, and participate in multidisciplinary rounds; educate patients on coping strategies, advance directives, and available support programs;
