Hudson Park Rehabilitation and Nursing has openings for some great, energetic and passionate Admissions Nurse (RN) to join our team. Hudson Park is a 200-bed skilled nursing facility on Northern Blvd in Albany.
Position
Description:
An Admission Nurse (RN)’s primary duties are to review admission documentation to establish care plan goals and perform assessments of newly admitted and re-admitted residents. Discharge Planning with patient and caregiver education for a safe transition home is an essential part of the role. Admission to Discharge, you are an important member of the Interdisciplinary Team. The nurse will provide direct care for all new admissions, review transfer documentation, review and verify physician's orders, and ensure a safe and successful admission process.
Responsibilities of an Admissions Nurse (RN):
- Ensures that the highest degree of quality patient care is maintained
- Assesses all potential admissions from the hospital prior to transfer to the facility ensuring appropriateness for transfer
- Performs a thorough and accurate medication reconciliation for all admissions
- Evaluates residents for changes in condition including exacerbation of chronic conditions
- Communicate findings with providers prior to hospital transfer
- Maintains proper infection control practices
- Tracks all admissions and discharges
Requirements of an Admissions Nurse (RN):
- A current NYS Registered Nurse license required
- Minimum 1 year of acute care clinical nursing experience
- Excellent physical assessment and critical thinking skills
- Excellent time management skills
- Ability to perform effectively in a fast-paced environment
- Excellent communication skills
