DescriptionFull Time Social Worker - Hospice Albany
The Community Hospice has a full-time opportunity for a social worker to join our hospice team caring for patients in homes and nursing facilities as part of our Interdisciplinary Team. As a member of the hospice team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff.
Position is Monday
- Friday 8a-4:30p.
- Diagnose and treat psychological, behavioral and emotional disorders as they relate to end of life care.
- Provide specialized skills and mental health therapy for clients.
- Identify barriers which impact optimal patient functioning.
- Ensure patients receive appropriate resources in most appropriate setting.
- NYS SW Licensure.
- Valid NYS Drivers license.
- Minimum 2-3 years experience ideally caring for seniors or those with advance illness.
- The Community Hospice provides a thorough orientation and a supportive work environment.

Pay Range$30.00 - $43.08
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
