Job Profile Summary
In the home health care setting, Licensed Practical Nurses (LPN) provide one‑on‑one skilled nursing care and treatment to patients in their homes. They assess patient needs, develop a plan of care with the physician, carry out the care plan with the patient, family, and support staff, and monitor and evaluate patient response to treatment, medication, and healing or mobilization progress. LPNs oversee case management and use a comprehensive, patient‑specific care plan with innovative solutions, spending quality time with the patient at home to promote health and independence.
Qualifications- Vocational / Technical Degree in Graduate of an accredited school of Practical Nursing (Required)
- 1 - 2 Years Practical Nursing Experience (Preferred)
Required Certifications / Licensures
- Licensed Practical Nurse (LPN) with current Georgia license, Basic Life Support (BLS)
Preferred Certifications / Licensures
- Advanced Cardiac Life Support (ACLS)
- Must possess and maintain a valid Georgia Driver's License
LPN - NURSING STANDARDS:
- Comply with nursing standards under the guidance of the Nursing Staff
- Maintain clinic, hospital policies, procedures, objectives, quality assurance, safety, environmental and infection control
- Interpret these to personnel, patients, and public as appropriate
- Follow established routine and utilize supplies and equipment in a cost‑effective manner
- Collects data in a systematic and ongoing process
- Issues are identified and diagnoses are developed based on assessment data
- Prioritizes data collection based on the patient’s condition and needs
- Identifies expected outcomes and plans based on diagnoses or issues
- Treats patients and families with compassion and implements plan of care
- Bases care decisions on plan of care
- Monitors and documents patient response
- Establishes rapport with clients
- Adapts communication style, listens and responds to patient needs
- Recognizes age‑specific responses to treatment
- Provides effective interventions
- Respects patients’ confidentiality and privacy
- Supports social, spiritual
