Job title:
Home Health Clinical Intake Specialist, RN
- 2 year / Associate Degree in Nursing from a state-accredited school (Required)
- 4 year / Bachelor's Degree in Nursing from a state accredited School of Professional Nursing (Preferred)
- 1 - 2 years General Medical Surgical Nursing Experience (Required)
- 1 - 2 years Hospice or Home Care Experience (Preferred)
- Registered Nurse (RN) with current Georgia license (Required)
- Basic Life Support (BLS) certification
- Must possess and maintain a valid Georgia Driver s License, reliable transportation, and current automobile liability insurance for travel to the patient s home
- Must furnish a report of Motor Vehicle Records for the past three (3) years
- Assessment and Diagnosis:
Collect data in a systematic and ongoing process; identify issues based on assessment data; prioritize data collection based on the patient s condition and needs; identify expected outcomes and plans based on diagnoses or issues - Care Planning and Implementation:
Develop an individualized plan with strategies addressing diagnoses; provide continuity through a plan that directs other team members; implement the plan in a safe and timely manner; document implementation and any changes; consult with healthcare providers and make referrals as appropriate; utilize community resources to implement the plan - Documentation:
Document and submit required information promptly; ensure documentation uses correct terminology, adheres to required style and format, and is tailored to readers/users - Outcomes and Evaluations:
Develop expected outcomes guiding continuity of care; involve patient/family and other disciplines; modify outcomes as patient condition changes; include patient and others in evaluation; evaluate the plan against patient responses and outcomes; document results - Quality and Ethics:
Use results of quality improvement activities in nursing practice; apply evidence-based knowledge to improve care delivery; participate in quality improvement activities; uphold nursing ethics and professional accountability; advocate for the patient, family, and community; maintain patient safety and contribute to a healthy work environment - Additional Duties:
Adhere to attendance guidelines; perform duties aligned with organizational values and policies; maintain current knowledge and attend ongoing education; attend meetings and complete required trainings on time; demonstrate age-appropriate care skills and customer service; wear protective equipment as appropriate
