Impact Wraparound Care Coordinator

A community-based health organization in Albany, Georgia is seeking an IC3 Care Coordinator to provide care coordination and case management services to youth and families. This role requires working with families to assemble Child and Family Teams, conducting intakes, and providing crisis intervention services. The ideal candidate will have a background in social services, strong organizational skills, and an understanding of community resources.
