Home Health Licensed Practical Nurse (LPN) – Full Time
Join Aveanna Healthcare as a Home Health Licensed Practical Nurse (LPN). This role requires you to deliver skilled nursing care, maintain accurate documentation, and uphold confidentiality and professional boundaries.
Position OverviewThe Licensed Practical Nurse is an essential team member responsible for providing and documenting skilled nursing care in accordance with the care plan and physicians’ orders for each patient while maintaining confidentiality standards.
Schedule: Monday – Friday visits with a shared on‑call rotation
Territory: Rogersville and surrounding area
- Deliver high‑quality patient care in compliance with physician orders under the direction of the Registered Nurse teammate.
- Provide hands‑on client care, complete documentation, and communicate timely updates with the client’s physician and other caregivers.
- Monitor changes in patient needs and responses, implement interventions to prevent or reduce risk.
- Educate clients and their family members based on specific needs.
- Use a variety of skills at patients’ homes, including phone triage, patient education, observation/assessment, wound care, infusions, catheter care, and PICC line dressing changes.

- Valid CPR certification and driver’s license in the state of application
- Medicare Skilled Nursing experience
- Basic understanding of Oasis
- 1 year of LPN experience in a home health or healthcare setting
- Additional home health or home‑care experience (home‑base) – plus
