Advanced Practice Provider – Hybrid Value‑Based Clinic at UAB Medicine Lee Branch Primary Care
Position DescriptionWork schedule:
Part‑time, 20 hours per week, Monday‑Friday flexible hours. No weekends/holidays/call.
The Lee Branch Primary Care team seeks a dynamic Advanced Practice Provider to pioneer a part‑time hybrid role focused on value‑based care delivery, combining remote telehealth and in‑clinic responsibilities, with an emphasis on Transitional Care Management (TCM) and Emergency Department (ED) follow‑up visits.
Responsibilities include timely patient outreach, medication reconciliation, coordinated follow‑up care, and meeting quality metrics to improve patient outcomes.
General ResponsibilitiesProvide medical care and treatment within the established scope of practice under collaboration with a licensed physician, coordinate plans of care, discharge, specialty referrals, health‑care education, comprehensive physical examinations, lab ordering, and performance‑improvement activities.
KeyDuties & Responsibilities
- Formulate and implement comprehensive patient care plans focusing on prevention, diagnosis, and treatment of disease, including patient/family education, prescriptions, lab and diagnostic testing, and therapies.
- Ensure complete documentation of patient assessments, care plans, and services provided.
- Order and interpret laboratory and diagnostic tests.
- Maintain records and reports required by regulatory agencies and The Joint Commission.
- Assist in coordination of research protocols and serve as patient advocate and liaison among health‑care providers.
- Provide orientation and training for new clinical personnel and conduct workshops and in‑service training.
- Serve as expert clinical resource to staff and patients in specialty areas.
- Provide a safe environment and demonstrate responsible professional growth.
- Perform other duties as assigned.
- Master’s degree in Nursing with Family NP or Adult Primary NP certification, or graduate of an accredited Physician Assistant program.
- No work experience substitution for education requirement.
Preferred 2 years of clinical APP experience.
Certified as an Advanced Practice Nurse (APN) with Certified Registered Nurse Practitioner (CRNP) Specialty by the Alabama Board of Nursing, or licensed as a Physician Assistant by the Alabama Board of Medical Examiners with PA‑C certification.
Required:
Basic Life Support (BLS) certification from the American Heart Association within 30 days of employment. Advanced Cardiac Life Support (ACLS) and/or Pediatric Advanced Life Support (PALS) may be required.
- Knowledge of procedures, protocols, and techniques to diagnose and treat health‑care conditions.
- Knowledge of and proficiency with computer systems and programs.
- Excellent written and verbal communication skills.
- Strong interpersonal and teamwork skills.
- Self‑directed and self‑motivated; able to multi‑task and problem‑solve.
- Maintain patient confidentiality.
