Overview

DDS Residential Program - Relief Per Diem Main Street role at The Bridge of Central Massachusetts. Relief Counselors support individuals diagnosed with a Developmental Disability to live as independently as possible within a group living environment. They provide guidance, support and perform activities of daily living, implementing procedures that ensure the health, safety, and well-being of individuals. They are strong advocates for individuals served and promote community integration.

Open Sky Community Services (the organization referenced in the original description) supports adults with developmental and intellectual challenges referred by the Department of Developmental Services. Our mission is to provide homes that are safe, nurturing, and address the individual’s personal needs and preferences. We encourage active roles in community life through volunteering, classes, outings, and health club participation. Our staff ensures individuals have the supports needed to lead meaningful, active lives.

• Assist in developing individualized, person-centered treatment plans and lead in daily implementation plans.
• Assist individuals to access and/or provide transportation.
• Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
• Review and complete all required documentation.

USD $17.50 per hour.

• High School Diploma, GED or equivalent required.
• Valid Driver’s License and acceptable driving record.
• Reliability, willingness to learn, and openness to new opportunities.
