The Case Manager assumes accountability and responsibility for managing the hospitalization of a caseload of patients from pre‑admission through one week post‑discharge.

• Part time – 24 hours/week
• Days 8a–4:30p
• Onsite

• Assumes accountability and responsibility for managing the hospitalization of a caseload of patients from pre‑admission through one week post‑discharge.
• Facilitates coordination of care by all members of the health care team to facilitate achievement of appropriate outcomes.
• Completes educational responsibilities.
• Performs leadership duties.
• Performs quality assurance and performance improvement duties.
• Performs communication and networking duties.
• Performs systems based practice duties.
• Demonstrates the knowledge and skills necessary to provide care for the physical, psych/social, educational, and safety needs of the patients served regardless of age.
• Other duties as required.

• Experience in case management is preferred.
• Experience working with all levels within an organization is required.
• Proficiency in MS Office (Outlook, Excel, Word) or similar software is required.
• Epic software or similar EMR software is required.

• Education:
  
  Bachelor’s degree in Nursing (BSN) is required;
  Master’s degree is preferred.
• Certification:
  Registered Nurse licensure is required.
• Current Health Care Provider BLS training from the American Heart Association is required.
• CCM or CCCTM Certification is preferred.
• Years of relevant experience: 3 to 5 years is required.
• Years of experience supervising:
  None.
