, our Wellness Director has the opportunity to make a real impact—ensuring individualized attention and continuity of care, especially as many of our residents have transitioned from our Independent Living setting. As Wellness Director, you will lead with compassion, clinical insight, and attention to detail—ensuring every resident receives the personalized care they deserve in a warm, supportive environment. Your role blends hands-on resident care, team leadership, and proactive wellness planning.
Resident Wellness & Care Coordination- Foster strong relationships by meeting regularly with each resident to conduct health and wellness checks
, answer medical questions, and provide peace of mind for both residents and their families. - Complete comprehensive wellness assessments prior to move-in, during condition changes, and at least semi-annually, ensuring compliance with both Cumberland Village policies and state regulations.
- Review each resident's pre-admission medical report to ensure their needs can be met safely and effectively within our community.
- Create and maintain individualized Negotiated Service Plans that reflect each resident's unique needs, preferences, and health goals.
- Oversee medication management for residents receiving assistance, including medication ordering, scheduling, and compliance with physician orders and regulatory standards.
- Serve as the first responder during on-site emergencies
, ensuring swift and appropriate clinical intervention while maintaining calm, compassionate communication with residents and their support systems. - Act as a liaison between residents, families, care teams, physicians, and third-party providers to coordinate holistic health services and ensure a continuum of care.
- Build and maintain positive partnerships with home health, hospice agencies, vendors, and physicians to expand the scope of supportive care available to our residents.
- Collaborate with the Food Services Director to ensure dietary needs and nutritional preferences are well understood and consistently met.
- Provide on-call support for urgent medical concerns and resident/staff emergencies, as part of a shared leadership rotation.
- Mentor, guide, and support resident care staff through ongoing training and development
, ensuring every team member is equipped to deliver compassionate, high-quality care. - Oversee infection control efforts, maintaining strict adherence to safety protocols and regulatory guidelines to protect residents and staff alike.
- Ensure all resident wellness files and documentation are accurate, up-to-date, and compliant with state and community standards.
- Collaborate with leadership to develop and maintain caregiver schedules that reflect both resident needs and staffing budgets—prioritizing consistency and coverage for exceptional care delivery.

- Licensed nurse (LPN or RN) in good standing in South Carolina
- Maintain CPR Certification
- 1+ years of leadership experience in senior living, assisted living, or a related healthcare setting
- Licensed nurse (LPN or RN) in good standing in South Carolina.
- Minimum of six months of recent experience as a professional nurse.
- Stand/walk up to eight hours a day. Sit up to two hours a day
- Frequently support up to 75 pounds. Occasionally lift/carry up to 50 pounds.
- Frequently kneel, bend, and reach.
- Secure proper assistance for transferring residents as needed
