Overview
The LTC Charles S. Kettles VA Medical Center is seeking a Full-Time Licensed Practical Nurse for the PACT/Sub-Specialty Clinics at the Adrian VA Clinic. This LPN is aligned under Chief of Staff Services and will be assigned to various Patient Aligned Care Teams (PACT) and/or Sub-Specialty Clinics located at the Adrian, Jackson, Toledo, and Findlay Community Based Outpatient Clinics (CBOCs).
Responsibilities- Be aligned under Chief of Staff Services and be assigned to various Patient Aligned Care Teams (PACT) and/or Sub-Specialty Clinics.
- Official duty station designated as Adrian CBOC.
- Support overall health care delivery efforts of the VA Ann Arbor Healthcare System (VAAAHS).
- May be assigned to one or more clinical areas within the community-based outpatient clinic (CBOC) and/or offsite ancillary clinic to perform clinical roles within the scope of practice (up to 25% of the time).
- Potential assignments to areas/clinics such as Adrian CBOC, Jackson CBOC, Toledo CBOC, and Findlay CBOC.
- Licensed Practical Nurse (LPN) in good standing.
- Ability to work with Patient Aligned Care Teams (PACT) and Sub-Specialty Clinics.
- Willingness to travel to multiple CBOCs as part of the role (up to 25% time).
