The ODVA RN Case Manager is a senior clinical role that elevates, supports and strengthens the admissions and care transition processes across ODVA’s State Veterans Homes. This position partners closely with Admissions Coordinators, facility leadership, and interdisciplinary teams to standardize outreach, improve candidate and resident selection, and ensure safe, timely, and appropriate placement of Veterans.
A core focus of this role is to improve the efficiency of reviewing, accommodating, and admitting Veterans with pending or incomplete applications by streamlining clinical review, accelerating decision-making, and reducing avoidable delays to admission. This position directly impacts access to care, mission fulfillment, and Veteran experience.
This role works in close collaboration with Central Office and facility leadership to advance ODVA’s strategic pillars of Outcomes, Ownership, Oversight, and Urgency.
Key Functions- Provide clinical case management support for admissions, transfers, and complex placement decisions.
- Standardize and reduce delay in admission review processes, by prioritizing timely review of pending applications.
- Expedite admission decisions and remove placement barriers.
- Facilitate interdisciplinary care coordination.
- Monitor admission timelines and identify bottlenecks.
- Ensure compliant documentation practices within the pre-admission and admissions process.
- Participate in re-hospitalization and delayed admission reduction initiatives.
- Maintain tracking tools and prepare leadership reports.
- Support survey readiness related to care transitions.

Monitor and report admission performance trends.
Urgency

Act decisively to prevent delays in access to care.
- Active Oklahoma RN license.
- Target BSN; MSN preferred.
- Five years progressive nursing experience.
- Long‑term care and admissions experience preferred.
- Strong verbal and written communication, organization, and time management skills.
- EMR proficiency;
Point Click Care experience preferred.
