Job Summary

As a member of the Population Health Management (PHM) Team, the Outpatient Case Manager works with members, providers and caregivers to provide intensive, comprehensive case management and increase efficient utilization of services for patients with complex needs. The role identifies chronic, complex and/or catastrophic cases and initiates intensive case management according to program guidelines. The PHM Op Case Manager effectively manages patients on an outpatient basis and during acute hospitalizations (in partnership with inpatient counterparts) to assure appropriate level‑of‑care, optimize safe transition to home, prevent re‑admissions and meet patients’ medical, environmental and psychosocial needs.

Responsibilities And Essential Functions
  • Implementation: Matches patient/family needs to available resources; utilizes telephonic and face‑to‑face communication; prioritizes collaboration with patients, families, and providers; coordinates care services and referrals; emphasizes wellness, disease management, and patient education; identifies care gaps and works with the team to close them; coordinates member visits with primary care providers and specialists.

  • Assessment: Reviews referrals; performs comprehensive assessment to identify patient/family needs; determines high‑risk areas; reviews all options/resources to promote optimum health.

  • Planning: Collaborates to form and modify individualized plans of care that address high‑risk patients with complex conditions; integrates evidence‑based guidelines; monitors patients across the health continuum and focuses on safe transitions to home, nursing home or rehab facilities.

  • Monitoring/Evaluation: Monitors care through data collection and analysis; evaluates effectiveness of the case management plan; manages performance metrics; participates in team meetings for process improvement and patient/provider satisfaction.

  • Performs other duties as assigned and complies with all Wellstar policies and code of conduct.

Required

Minimum Education
  • Associate’s Nursing or Diploma (RN) or Bachelor's Nursing (Preferred)
Required Minimum License(s) And Certification(s)
  • RN – Reg Nurse (Single State) or RN‑COMPACT – Multi‑state Compact
  • BLS – Basic Life Support or ARC‑BLS – American Red Cross Basic Life Support or BLS‑I – Basic Life Support – Instructor
Additional License(s) And Certification(s)
  • Case Manager Certification (CCM) (Preferred)
  • Computer experience with Microsoft Office suite and electronic health records (Preferred)
  • Experience in data collection and analysis and basic research techniques (Desired)
Required Minimum Experience
  • Minimum 5 years of clinical experience
  • Case Manager certification (CCM) Preferred
  • Experience with data collection and analysis
Required Minimum Skills
  • Knowledge of complex case management role and processes
  • Customer‑focused interpersonal skills to interact with practitioners, multidisciplinary teams, community agencies, patients and families from diverse backgrounds
  • Outgoing and autonomous, flexible personality that can engage the geriatric population over the phone and support the development of PHM CM role
  • Leadership qualities including excellent organization, time management, verbal and written communication, problem‑solving, decision‑making, priority setting, and delegation
  • Ability to utilize risk‑stratification screening criteria and review clinical data to identify patient needs
