Description

CSPNJ, a mental health nonprofit agency, is looking for a Service Intake Coordinator. Under the direction of the Clinical Plan Coordinator, the Service Intake Coordinator is responsible for the coordination and tracking of all referrals and intakes assigned to the region and develops relationships with referral sources, government agencies, linkage agencies, property managers, etc.

Responsibilities
  • Direct face-to-face billable contact with clients (55%–60%); remaining time spent on training, developing relationships, intake coordination, and managing referrals.
  • Complete progress notes conjointly at the time of service and comply with regulatory standards.
  • Manage initial client entry, serving as the first point of contact to gather essential information, screen for service appropriateness, verify benefits, and ensure smooth transition to care.
  • Identify client needs, troubleshoot issues, and connect clients to appropriate community resources.
  • Conduct duties of a Support Worker including maintaining a caseload of persons served.
Requirements
  • Bachelor’s degree in a mental health/human service discipline or equivalent.
  • The ability to write clear, concise, and accurate billable progress notes.
  • Must have a valid NJ Driver’s License with acceptable driving record.
  • Exceptional time management skills to support the needs of multiple residents.
  • Excellent communication, assessment, and people skills.
  • Patience and emotional resilience; ability to remain calm under pressure when dealing with challenging situations.
  • Ability to carry out domestic duties.
  • Ability to work with individuals with mental illness in a caring and professional manner.
  • Proficiency with MS Office and general computer literacy.
  • Bilingual skills are a plus.

Salary

Hourly Rate: $19.23 - $22.12 based on education & experience
